We are looking for someone to act as a source of expertise on the assessment and management of patients presenting with a wide range of pathologies to physiotherapists, student physiotherapists, skilled non-registered staff, other healthcare professionals, GPs, Consultants, patients and their carers.
Able to provide a high standard of physiotherapy service within Torbay. This will involve community rehabilitation, crisis response and Intermediate care cover. You will be required to work between these areas depending on the needs of the service, which will include participation in the Baywide Intermediate Care weekend rota and late duty rota within the zone. This may also include, from time to time, covering other localities within the Trust.

Main duties of the job
* To deliver high quality care to the housebound patients of Torbay and work as part of a multi-disciplinary team and in partnership with primary care and secondary care.
* To undertake all aspects of clinical duties as an autonomous practitioner, including professional and legal accountability and managing clinical risk for all aspects of own work. Some aspects of work will involve delivery of care as part of a multidisciplinary team. In most cases to be the sole health professional with responsibility for that patient's episode of care.
* To undertake advanced physiotherapeutic assessment of patients with diverse or complex physical, psychological, cognitive and behavioural conditions in order to provide an accurate diagnosis and prognosis.
* To formulate and deliver individualised physiotherapy treatment programmes, including manual therapy techniques, therapeutic handling, patient education, electrotherapy, provision of exercise (individual/group), and other alternative options. This includes the appropriate selection of referrals for inclusion in a range of treatment programmes, one to one sessions using advanced treatment options, advanced communication, questioning and reasoning skills.

Qualifications and training
Essential
* Diploma/degree in physiotherapy State registration - current HCPC certificate.
* Experience within the NHS including experience as a junior in a broad rotational post covering a range of experience in orthopaedics, elderly care, medical, surgical, neurological and musculoskeletal conditions.
* Evidence of CPD maintained in a portfolio, including attendance at recent Postgraduate courses relevant to the clinical field.
Desirable
* Postgraduate training/experience in rehabilitation and elderly care.
Knowledge and experience
Essential
* Working knowledge of a broad range of musculoskeletal, neurological and medical conditions common in the elderly.
* Knowledge of the relevant NSFs and clinical guidelines.
* Experience of working as part of a multidisciplinary team.
* Experience supervising student physiotherapists or physiotherapy assistant/technical instructors/support workers.
* Some experience working with a biopsychosocial approach to assessment and treatment.
* Experience of working in inpatient, outpatient and community settings.
Desirable
* Experience of primary care rehabilitation.
* Knowledge of the safeguarding adults process.
Specific Skills
Essential
* An understanding of Clinical Governance and its implications for services, including experience of quality issues and participation in research and audit.
* Competent IT skills.

* Ability to work alone assessing and managing risk. Clinical workload involves daily/weekly/monthly domiciliary visits with telephone support from the physiotherapy lead if required, and lone working during the out of hours service.
* Ability to be mobile in the South Devon area to complete community visits and attend meetings.
