As part of the Berkshire West Community Respiratory Service, you will providehigh quality,evidence-basedcare for patientsundertakingCardiac orPulmonaryRehabilitation.The roleincludes assessing, planning, implementing,monitoringand evaluatingexercise programmes according to the needs of the referred individuals. They will be based in the community (at leisure centres/community centres or at the client’s home) as a lone worker and with other members of the multidisciplinary team.
As a team player,you will be workingwithin theCardiac and Respiratory Specialist Services (CARSS) including theRespiratory Services (and Pulmonary Rehabilitation)Heart Function Service and Cardiac Rehabilitation, all based at Coley Clinic. Your energy and drive will be rewarded with the support of a dedicated team that strives to create a progressive,friendlyand considerate environment.
• Work as part of an integrated teamproviding community basedCardiac andPulmonary Rehabilitation.
• Work as part of theCardiac andPulmonary Rehabilitation teamtodeliver therehab programmes including assessment of patients and supervision of groupexercise classes.
• Toprovide Home Exercise Programmes for those who are unable to attend thecommunity basedprogramme
