What you will do: Conduct assessments of patients within 72 hours after their discharge from hospital admissions. Coordinate with various agencies and provide necessary equipment from your car to aid patient recovery. Be punctual with your attendance, starting at base by 8am and concluding your duties by 4pm at base or leaving the patient's property. Efficiently complete patient notes on a laptop, ideally during patient interaction for accuracy and efficiency. Possibly work during the weekends, providing you with opportunities to earn even more. Requirements: HCPC-registered Occupational Therapist. Minimum 2 years of experience in a relevant community or hospital discharge setting. Car driver with access to own vehicle - preferred for this role due to travel requirements. Proficiency using SystmOne electronic records system.
