Community Assistant Practitioner - Diabetes

The Norfolk Community Health and Care NHS Trust Diabetes Team is primarily based in West Norfolk. We are a growing, friendly, and supportive team who provide care for people living with diabetes in a variety of settings including clinics, patient's homes and care settings. As part of an innovation, a fantastic opportunity has arisen to work within the NCHC Diabetes Team. We have two positions to fill at Band 4 for 18.75hrs per week. You will be working in either Norwich or North Norfolk alongside a Band 6 Diabetes Specialist Nurse and under the supervision of the Diabetes Specialist Nurse Lead for NCHC. Please specify within your application your 1st and 2nd preference for North Norfolk or Norwich.
Main duties of the job
You will work within the Diabetes Team providing knowledge and skills, managing your own caseload assigned by the Diabetes Specialist Nurse. Responsibilities include:
* Carrying out diabetes reviews, venepuncture, foot checks, education, and continuous glucose monitoring and application.
* Providing health education to service users, their families, carers, and other agencies involved in their care management.
* Contributing to the delivery of high standards of health care by assessing, planning, implementing, and evaluating packages of care.
* Working under supervision to assess and review patients with diabetes to optimise treatment regimes.

Job responsibilities
Clinical
* Assess patients with straightforward requirements based on predetermined department protocols.
* Prepare for, carry out and monitor assessments and treatments in specified clinical areas.
* Apply competency-based treatment techniques/care to patients under supervision.
* Modify and progress intervention/treatment using clinical reasoning.
* Monitor patients and alert a qualified practitioner of any unexpected changes.
* Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals.

Person Specification
Qualifications
* NVQ 3 or equivalent knowledge and experience.
* Foundation degree or equivalent demonstrable knowledge and skills.
* Experience in health or social care settings.

* Effective communication in written and verbal English.
* Full UK driving license and access to a vehicle.
* Professional appearance and reliability.
