This exciting post will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.
We are looking to recruit a Band 6 Memory Assessment Nurse.
The posts will work in the areas with the highest waiting times and will work with colleagues to reduce the waiting times in that area. There will be a requirement to travel between sites throughout Nottinghamshire.
Main duties of the job
* To work as part of the multi-disciplinary team to screen for appropriate referrals
* To complete an assessment of clinical presentation and symptoms
* To share information amongst the multi-disciplinary team and create appropriate and person-centered care plans
* To refer for appropriate diagnostic procedures e.g., scans, neuropsychology, occupational therapy.
* To make independent diagnostic decisions based on consultation, history, presentation, pathology, and/or imaging results and to seek supervision from service medical support in complex diagnostic situations.
* To complete accurate records

Job responsibilities
This role is working within Nottinghamshire Healthcare NHS Foundation Trust within the Mental Health Services for Older People directorate.
The successful candidate will work within the MAS service and will review/accept referrals for individuals over the age of 65 with cognitive impairment, completing assessments for the same to ascertain whether or not a Dementia diagnosis is appropriate.
* To screen referrals for appropriateness
* To undertake initial assessments and create a treatment plan
* To liaise with the MDT to explore symptoms and causes
* To signpost for post-diagnostic support
Person Specification
Qualifications
* Must hold RNMH qualification
Training
* Education to master's degree level or equivalent experience
* Clinical supervision course
* Teaching and assessing training
Experience
* Substantial experience of MDT working
* CPD relevant to current post
* Current knowledge and understanding of older people's mental health needs and ability to demonstrate clinical competence
* Substantial and considerable experience in assessing and prescribing for people with dementia
* Can evidence knowledge of dementia
* Community Mental Health experience
* Knowledge of Trust non-medical policy and local procedure
* Relevant clinical courses facilitated research and audits that contribute to governance, service development, and service planning
* Evidence of leadership skills and development project work.
* Effective teaching and presentation skills
* Highly specialised range of interpersonal and communication skills.
* Effective time management and organisational skills. Caseload management
* Basic IT skills
* Ability to organise and prioritise workload.
* Clinical supervision skills Ability to manage change
Contractual Requirements
* A full UK driving licence and vehicle for business use is required for this post
