Advanced Nurse Practitioner/ Advanced Clinical Practitioner (LD)
Taunton Vale Healthcare is dedicated to providing exceptional care for their patients, including those with learning disabilities and complex physical health needs. We support 18 specialist homes for individuals with learning disabilities, offering compassionate, patient-centred care.
We are seeking an experienced and enthusiastic clinician (Advanced Nurse Practitioner, Advanced Clinical Practitioner) who has a passion for working with patients with learning disabilities. This role involves leading and responding to the daily healthcare needs of residents across our specialist homes, patients within the community, and those in Supported Living. You will be pivotal in ensuring the delivery of high-quality, tailored care that promotes well-being and independence.
Main duties of the job
* Act as the clinical lead for patients with Learning Disabilities within the community, within Supported Living, and across the 18 specialist learning disability homes, overseeing and coordinating care delivery.
* Respond to the daily healthcare needs of residents, ensuring timely and effective management of their medical conditions.
* Ensure annual Health Checks are completed in conjunction with junior staff members.
* Provide clinical expertise in managing patients with learning disabilities and complex physical health needs.
* Conduct assessments, develop care plans, and review treatment strategies for residents in collaboration with the multidisciplinary team, the Learning Disability Liaison Team, and the Community Learning Disability team.
* Act as an independent prescriber, ensuring safe and effective medication management.
* Engage with families, carers, and other professionals to provide holistic and compassionate care.
* Stay updated on best practices and contribute to service development to enhance care quality.

Person Specification
Qualifications
* Registered as an Advanced Nurse Practitioner, or Advanced Care Practitioner with relevant professional body (e.g., GMC, NMC, or HCPC).
* Experience or interest in contributing to service development and improvement initiatives.
* Flexibility to combine the learning disability care home role with other primary care responsibilities.
Experience
* Extensive experience in caring for patients with learning and physical disabilities.
* Independent prescribing qualification and experience.
* Experience of working in primary care.
* Experience of working in a GP practice.
