Requirements:
* Manage a complex caseload, assessing, planning, and reviewing individual needs to promote independence.
* Conduct assessments using various health and social care tools, including continuing healthcare evaluations.
* Identify and facilitate access to Reablement services, promoting a Strength-Based Approach to enhance independence and well-being.
* Utilise community resources and individual strengths to maximise preventative support and delay long-term care needs.
* Coordinate ongoing service provision, ensuring effective collaboration, monitoring outcomes, and undertaking safeguarding responsibilities.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
