We are recruiting for a Lead Physical Health Nurse to join our Mental Health teams at the Whiteleaf Centre in Aylesbury where you will be working with our Adult and Older Adult Inpatient services to deliver high quality care.
This is a great opportunity for a registered General Nurse/Dual Qualified Nurse (RMN/RGN) with excellent experience to support teams deliver safe and excellent physical health care for patients with complex mental health problems. You'll be instrumental in driving the physical health agenda across the mental health directorate, facilitate training and work collaboratively with services to provide good holistic care. You'll be a positive role model to our teams, working closely within the Multi-Disciplinary Team and be integral in decision making that improves physical health outcomes for patients. You'll also be involved in Quality Improvement work across the wards to improve overall service provision.
This is a full-time role based at the Whiteleaf Centre in Aylesbury Buckinghamshire.
Main duties of the job
1. Adviseandcontributetothedelivery ofhigh-qualitynursingcareforpatientswithphysicalhealthissues withintheA&OA wards.
2. Encourageanddevelopinnovativepracticethatwill improvetheoverall healthandwell-beingofindividualswithmentalhealthproblems whousetheTrust services.
3. Leadonthedevelopmentanddeliveryofhigh-qualitynursingcareplansforpatientswithSeriousMentalIllness whichadvise onphysicalhealthcareincluding smoking cessation,diet,weightgain,exercise,lifestyle,sexualhealth,andsideeffect monitoring.
4. Develop,co-ordinateandcontributetothedelivery of physicalhealthandwell-beingtrainingprogrammesandeducationalmaterials forstaff.
5. Partakeandleadonauditsandresearchinvolvingthephysicalwellbeingof patients throughouttheservice.
6. Leadondevelopingandimplementinga healthpromotionstrategy withintheservice. Identifyappropriatetools tomeasuretheeffectivenessofwell-beingsupportandhealthpromotionprogrammes.
7. Provideleadershiptophysicalhealthleadswithineachward and community services.

Detailed job description and main responsibilities
· Provide professional clinical leadership, working in collaboration with the appropriate multidisciplinary groups to meet the physical health needs of patients within the adult and older adult inpatient wards.
· Provide specialist evidenced based knowledge and clinical skills to the staff and patients within the service to ensure that the physical health and well-being needs of individuals is met.
· Lead on the strategic development and delivery of health promotion and physical health care within the service.
· Develop and deliver, alongside the clinical practice educators, training on physical health and health promotion to clinicians within the service as required.
· Develop and maintain good links with other services, such as physiotherapy as required.
· Work collaboratively with other clinical nurse leads for physical health within the trust.
· Contribute to advanced practice initiatives in nursing by contributing to education and training with links with the learning and development department and higher education establishments.
· Provide professional and managerial supervision to other physical health nursing roles and managerial supervision to other health professionals working within the physical health team.
· Provide leadership to a team of physical health leads within each ward.
· Lead on clinical audit in relation to physical health. Contribute to research as required.
· Give advice, guidance and provide interventions for patients with complex physical health needs.
· Provide assurance that patients in the service have equitable access to NHS screening progress as those in the community.
