At LGT we are currently seeking a highly motivated, resourceful pharmacist to join our energetic and well-motivated team to lead on the creation and provision of a Pharmacist led Biologics Hub within specialist Medicine at the Queen Elizabeth Hospital (QEH).
The role will involve establishing a pharmacist led hub that will centralise the prescribing of homecare medicines for dermatology, neurology, rheumatology and gastroenterology at LGT for stable patients.
The successful applicant will also contribute to the pharmacy service offered to specialist medicine at the QEH site, including representing the Trust at regional integrated medicines optimisation committee meetings, supporting the roll out of biosimilar switching programmes, participating in MDTs, engaging with commissioners and ensuring clinically effective and cost efficient use of drugs.
The ability to provide support and provide education to the wider clinical pharmacy team and other healthcare professionals is fundamental to this post.
Main duties of the job
The successful candidate will be expected to lead on the pharmacy aspects of biologic medication in the Trust, with a focus on prescribing biologics for established stable patients and facilitating timely biosimilar switches. Other aspects of the role will involve implementing policies and procedures to ensure safe and effective medication management, providing education and training to staff and patients on safe and effective medication use and to participate in research and quality improvement initiatives to improve patient outcomes.
The successful candidate will be a highly motivated team player with excellent problem-solving & time management skills. They will be responsible for supporting the delivery and development of a patient focused clinical pharmacy service Trust wide and assisting in the promotion of the cost effective use of medicines, as well as identifying and delivering drug cost savings and achievement of any national CQUIN or iURP programmes as necessary.
Applicants must be currently registered with the GPhC as a pharmacist and completed the clinical pharmacist diploma. An independent prescribing qualification or working towards one is essential. Successful candidates will have excellent communication and interpersonal skills, be able to engage with key stakeholders throughout the Trust. The post-holder must be able to represent the Trust at relevant offsite network or regional meetings as required.

Job Summary:
1. To be responsible for the coordination and delivery of a responsive, proactive, patient-focused clinical pharmacy service to clinical Divisions across the Trust within defined area. This includes the promotion of safe, effective and cost effective drug use to facilitate patient admission and enhance continuity of care across the primary and secondary care interface.
2. To provide support and direction to the band 7s in Medicine cross site.
3. To act as a role model and provide a consistently high standard of pharmaceutical service to patients in line with the Trust and Departmental objectives.
4. To be the main pharmacy contact for Dermatology and Neurology within the clinical divisions.
5. To support Dermatology and Neurology in managing their drugs budget. Support service development that involves medicines or pharmacy services in these directorates.
6. To provide operational and clinical support to the pharmacy department as dictated by the needs of the clinical pharmacy service across the Trust. This includes supervision, training and support to junior pharmacists, trainee pharmacists and other pharmacy staff as necessary.

Key Result Areas & Performance:
Clinical Services and Patient Care
1. To participate in the front-line provision of Pharmacy Services as appropriate and when To act as a clinical pharmacist in an appropriate area and participate in consultant ward rounds as required.
2. To be registered with the General Pharmaceutical Council as a pharmacist independent prescriber.
3. To practice as a pharmacist independent prescriber, where appropriate to improve patient care and safety, and to continue to develop the role of pharmacist independent prescriber within the specialty.

This includes undertaking medicines reconciliation, assessing pharmaceutical needs, assessing and facilitating the use of patients own drugs, ensuring appropriate drug supply, maintaining both the one stop dispensing system and ward based dispensing service by pharmacy
6. To participate in consultant post-take ward rounds to identify and resolve pharmaceutical care needs of patients and ensure that any follow up plans are shared with the ward based pharmacy team and medical staff as

2. To provide the Director of Pharmacy with details of output, performance and activity at specified time intervals in a format agreed with the Director of Pharmacy.
3. To identify resources required to improve pharmacy services within the specified directorates.
4. To support the development, implementation and adherence to local and national guidelines/policies to ensure cost-effective and safe prescribing within the division.
5. To collaborate in the development of Integrated Care Pathways, Standard Operating Procedures and Patient Group Directions within the division as necessary.
