Purpose
To deliver high-level, patient centred, evidence-based care, whilst integrating advanced clinical skills, leadership, education and research into practice.
KEY RESPONSIBILITIES
Direct Provision of High-Quality Specialist Palliative Care
Assessment, diagnosis, and management of patients under the care of the Hospice. Majority of work is based in the adult in-patient unit, but if required as outpatients or domiciliary visits. Work includes:
* Using effective joint working and prioritisation to support responsive admissions within available capacity each day.
* Supporting safe transfer of care from acute or community settings to hospice settings by in-depth assessment and co-ordination.
* Admission assessment and forming a management plan, including receiving patients with undifferentiated and undiagnosed problems.
* Providing explanation/information according to individual patient preference, communication with patients/family and other professional colleagues.
* Arranging and reviewing appropriate investigations.
* Be responsible for assessing and recognising emergencies in palliative care.
* Interpreting information and taking appropriate action, supporting other team members.
* Supporting and enabling patients and carers to make informed decisions regarding their treatment and management.
* Safe and effective prescribing in accordance with standards.
* Supporting advance care planning and managing challenging symptoms.
* Providing effective documentation and handover.
* Managing expectations and providing high quality responsive care in accordance with current evidence and accepted practice.
* Working within the medical team to maintain continuity of care.
* Assessment of appropriateness of discharge, contributing to discharge and aftercare planning.
* Liaising promptly with professional colleagues involved in patient and family care during and after admission.
* Encouraging and managing appropriate referrals within the specialty, attending complex case discussions and MDT meetings.
* Using excellent prioritising and time management skills to meet the conflicting needs of the service.
Provision of Specialist Palliative Care Advice and Multi-disciplinary Team Working
Working with professionals, patients and carers includes:
* Decision making over remote advice regarding symptom management and appropriateness of admission or discharge.
* Timely completion of medical correspondence.
* Working with the multidisciplinary team to provide holistic specialist palliative care.
* Ensuring appropriate involvement of relevant professionals and effective joint working.
* Promoting a rehabilitative palliative care approach.
Leadership and Strategy
Work autonomously across professional boundaries, developing and sustaining partnerships to improve health and healthcare delivery systems. Responsibilities include:
* Providing clinical leadership and contributing to quality improvement activities.
* Participating in incident investigation and developing action plans for improvement.
* Negotiating and facilitating change in practice for quality clinical services.
* Implementing new policies and clinical guidance with nursing and medical staff.
* Supporting and facilitating development of clinical skills for nurses and leadership.
* Maintaining effective communication with multi-disciplinary teams, patients, and external agencies.
Education and Research
Responsibilities include:
* Training and supervision of trainee advanced clinical practitioners, foundation doctors, and GP registrars.
* Planning and delivering interventions for the learning needs of medical students and other professions.
* Engaging in professional development to aid progressive practice policies.
* Leading and participating in research studies and ensuring quality standards are met.
* Developing and delivering teaching sessions for students and colleagues.
Advocate for a culture of continuous learning and evidence-based practice.
