Job Responsibilities
1. Provide the first point of contact for clinical queries from practice receptionists.
2. Be responsible for telephone and face-to-face triaging.
3. Undertake urgent/same day/duty/out-of-hours clinics according to time schedule.
4. Undertake home visits.
5. Assist with vaccination programmes (including mandatory training, completing relevant administration and delivery of the vaccines).
6. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated or complex diagnosis under the supervision of the duty clinician.
7. Identify red flags and underlying serious pathology and take appropriate action.
8. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
9. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan.
10. Initiate and interpret tests and investigations as appropriate.
11. Initiate a management plan to include medication, assess for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
12. Actively take a personalised care approach to enable shared decision making with the patient.
13. Work with patients in order to support compliance with and adherence to prescribed treatments.
14. Provide information and advice on prescribed or over-the-counter medication including side-effects and interactions.
15. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
16. Assess, identify and refer patients presenting with mental health needs.
17. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
18. Signpost appropriately to other services in the identified healthcare pathway.
19. Record accurate data in the patient records to an agreed standard.
20. Collaborate with other members of the Primary Health Care Team including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care.
21. Referral to and liaison with multi-professional teams, including social care services and secondary care providers, within locally agreed healthcare pathways.
22. Ensure a high standard of clinical practice is maintained by ensuring care is based on current best practice, embarking upon further training if required and continuing to update clinical knowledge.
23. Assisting in completing Quality and Outcomes Framework (QOF) targets and Local Commissioning Scheme (LCS) targets.
24. Administrative duties such as Docman and 111 reports.
25. Work across sites/practices to deliver Federation and PCN objectives.
26. Contribute to audit and research projects.
27. Contribute to education and supervision within their scope of practice for the multi-professional team.
28. Complete mandatory training.
29. Abide by the Information Governance NHS England guidelines on handling patient records.
30. Be registered with a GP.
Quality Requirements
1. Recognise and work within own competence and professional code of conduct regulated by the HCPC and College of Paramedics.
2. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
3. Be aware of data protection (GDPR) and confidentiality issues particularly within a GP surgery.
4. Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.
5. Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
6. Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
7. Deliver care according to NSF, NICE guidelines and evidence-based care.
8. Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
9. Keep up to date with current evidence-based practice.
10. Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
11. In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
12. Support and participate in shared learning across the practice and wider organisation.
13. Assist in the development and implementation of policies and procedures.
14. Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients health procedures and local guidance.
15. Attend and participate in Significant Event Audits.
16. Attend practice meetings.
17. Work within the HCPC code of professional practice and Scope of professional conduct, H&S at work Act 1974, Data Protection Act 1984 and Access to Health Records 1990.
Health and Safety
1. Under the Health and Safety at Work Act 1974, as an employee, you must take reasonable care for the health and safety of yourself and for other persons who may be affected by your acts or omissions at work.
2. The Act also states that you must not intentionally or recklessly interfere with or misuse anything provided in the interests of health, safety and welfare.
3. You will assist in promoting and maintaining your own and others health, safety and security as set out in applicable Practice policies and protocols.
This will include:
1. Using personal security systems within the workplace as directed.
2. Identifying risks involved in work activities and managing those risks.
3. Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
4. Reporting health and safety hazards and infection hazards immediately when recognised.
5. Keeping own work areas and general/patient areas clean and assisting in the maintenance of general standards of cleanliness.
