Main duties of the job
Participate in consultant ward rounds/MDTs to improve medicines optimisation by improving and ensuring safe, cost-effective prescribing and ensuring compliance with Trust prescribing guidelines and policies amongst other duties.
Work alongside the Clinical Pharmacy Technicians to implement and deliver a 28-day one stop dispensing service, the efficient use of patients own medicines and promote self-administration of medicines.
Support ward staff to ensure that adequate ward pharmacy stock levels are maintained on allocated wards and to regularly review and adjust levels if necessary, depending on prescribing trends.
Involvement in regular audit work in defined areas of practise, to help further improve standards of care for patients of the Trust.

Person Specification
Experience
* Experience of working in a hospital setting (Community and/or Mental Health)
Qualifications
* Masters Degree in Pharmacy plus successful completion of pre-registration training year
* Member of General Pharmaceutical Council (GPhC)
* Diploma/MSc in Clinical Pharmacy or equivalent
