Working across the Five Elms PCN practices to coordinate patient care, the post holder is to provide time and support to patients, acting as a single point of contact to enable them to navigate the healthcare system. In this way, you will provide a more joined-up and co-ordinated care journey for the patient. You will work closely with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, ensuring that appropriate support is made available to them and their carers, and that their changing needs are addressed.
Main duties of the job
The main duties include:
1. Proactively identify and work with a cohort of people to support their personalised care requirements.
2. Provide coordination and navigation with the aid of digital tools for people and their carers across health and care services.
3. Work closely and in partnership with the Social Prescribing Link Workers.
4. Bring together a person's identified care and support needs and what matters to them; explore options to address these in a single personalised care and support plan.
5. Help people manage their needs, answer queries, and support them to navigate their appointments.
6. Raise awareness of shared decision-making and decision support tools, and assist people in being more prepared for shared decision-making conversations.
7. Ensure that people have high-quality health information to help them make informed choices about their care.
8. Assist people in accessing self-management education courses, peer support, or interventions that support their health and wellbeing.
9. Participate in clinical governance activities and contribute to improving health outcomes through audits, risk management, and quality improvement.
10. Provide coordination and navigation for individuals and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care roles.
11. Support the coordination and delivery of multidisciplinary teams within PCNs.
About us
Five Elms PCN is seeking to employ a Care Coordinator. The post holder will work closely with GP Practice Managers, staff, GPs, and allied community health professionals. The post holder will be employed by Five Elms PCN Ltd, which is the host organisation providing support for these roles, and can expect to be based in practices. Management support will be provided by the PCN Manager.

Qualifications
* Good standard of general education (GCSEs, NVQ, BTEC) and competent in Microsoft Office.
* Experience using the EMIS web clinical system would be beneficial.
* Previous primary care experience.
