Clinical Support Worker Higher Level - Therapy Assistant (UCR Therapy)

This exciting role has been developed to support the urgent community response multidisciplinary team working across the South Tees locality.
You will need to be able to work on your own; as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed. You will work within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses.
Applicants must have access to a vehicle for work purposes to meet the requirements of the role.
Main duties of the job
This is an opportunity for those who are interested in working in the local community and being part of an exciting new model of working in the community localities of Middlesbrough, Redcar & Cleveland.
Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post.
This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to help bridge the gap between hospital and home. As a senior integrated support assistant you will provide both rehabilitation and nursing care following a person centred plan within the patients' own home.

Qualifications
* 3 A-levels OR Health and Science T-level OR equivalent level 3 qualification(s) and Functional Skills (level 2) OR As a level 3 apprenticeship, such as the Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway) AND/OR Short courses and completion of competency workbook equivalent to Level 3 in a range of patient and clinical care duties AND/OR Equivalent knowledge and experience in a range of clinical care duties equivalent to Level 3; - Theoretical knowledge - Experience of a range of care and related procedures - clinical observations - knowledge of relevant legislation
* Care Certificate
Experience
* Experience of working in health or social care.
* Experience of undertaking and recording clinical observations and escalation of changes or findings.
* Experience of working as a therapy assistant.
Knowledge & Experience
* Excellent interpersonal, communication and active listening skills; Ability to adopt appropriate styles and methods of communication.
* Analytical and judgement skills to make decisions based on patient/service user interactions and observations and metrics.
* Time-keeping, planning and prioritisation skills.
* Basic IT skills to use electronic patient records and other Trust systems.
* Working knowledge of legislation underpinning care provision.
* Understanding of the importance of robust, accurate, clear, timely documentation.
* Knowledge of Safeguarding.
* Awareness of GDPR and the need for confidentiality.
* Working knowledge of Occupational Therapy or Physiotherapy.
