Advanced Clinical Practitioner - Hospital at Home

As University Hospitals Dorset, we are a three-site acute trust with ‘university hospital’ status through our partnership with Bournemouth University, working with an annual budget of £730m.

This is an exciting opportunity for an Advanced Clinical Practitioner to join and assist with the development of the Hospital at Home service, alongside virtual wards specialities within Older Persons Services.
The role will include assessments, diagnostics and implementing a treatment plan whereby patients' conditions and plan of care can be met within their own home or within community settings to prevent them from remaining in acute settings when deemed appropriate.
Use of a car for transportation is essential.
We are looking for a dynamic, compassionate, organised and confident person who has experience in managing acutely unwell patients as there will be those situations when patients require acute assessment, care and escalation.
You will need to demonstrate the necessary skills to deliver services to patients with a wide range of complex clinical needs including frailty.
This role requires an active UK NMC Registration
Based Location: Christchurch Day Hospital but covering the whole of BCP area travelling

Person specification
Knowledge
* Demonstrable theoretical and practical knowledge of OPS specialty common presentations.
* Conversant in older persons Advanced practice developments at national/local levels.
* Legal/professional/ethical issues of advanced practice.
* Advanced clinical skills, such as assessing patient’s physical status requiring precision, accuracy, dexterity.
* Demonstrable ability to work in a busy environment and resilience when the service is under pressure.
* Ability to analyse and interpret complex information from patient assessment and interventions.
* Ability to work unsupervised and on own initiative and as part of a multi-disciplinary team.
Experience
* Demonstrable experience of working independently at a Band 7 Clinical role within relevant specialty.
* Experience of leading & developing people.
* Experience of teaching, mentoring and developing staff.
* Managing defined caseload of patients.
* Experience of working across organisational boundaries.
* Publication/Conference Experience.
* Team leader.
Qualifications
* Professional registration with relevant professional body.
* Recognised teaching or mentoring qualification.
* Masters level Advanced Clinical Practitioner/Equivalent Masters.
* Non-Medical Prescribing.
* Budget Management.
* Recognition as Advanced Practitioner by the National Centre for Advancing Practice.
Personal Attributes
* Excellent interpersonal and communication skills, using a range of methods.
* Able to deal with demands of rapidly developing and changing service.
* Self-motivated and reliable.
Other requirements specific to the role
* The use of a car for transportation to visit patients at home is essential, as well as the willingness to obtain business car insurance.
