Job overview
Sutton Health and Care (SHC) Reablement Unit aims to support patients to regain function and independence prior to discharge by providing intensive multidisciplinary rehabilitation on the Reablement Unit. The activity coordinator will lead on developing a patient-centred activity programme, facilitating groups and individual interventions to enhance their quality of life and contribute to their ongoing rehabilitation goals. The post holder will work as a member of the multidisciplinary team, liaising closely with nursing, therapy staff, and reablement assistants.
Main duties of the job
1. Work closely with the MDT in identifying and facilitating patient-centred activities.
2. Prepare and implement social, leisure, and recreational activities with patients.
3. Arrange individual activities for patients, providing 1-1 social contact.
4. Provide culturally appropriate activities for patients.
5. Ensure a range of individual activities for patients; e.g. board games, electronic games, music therapy, creative crafts, and reading materials.
6. Organise social events on the Reablement Unit.
7. Organise and arrange group activities for patients to promote social interaction.

Experience
