HDFT 0-19 Children's Community Public Health Services have an exciting opportunity to help you to progress your career to become a Registered Adult Nurse. In collaboration with Sunderland University, we can now offer the chance for you to start and complete the Registered Nurse Degree Apprenticeship. We have 2 positions to offer with base areas in our 0-19 children's community services and settings, with the acute ward/department placements required to achieve the relevant competencies and requirements of the course facilitated in regional Acute Hospitals with the relevant partnership arrangements in place. Base area to be determined, however will be within the HDFT footprint of community services which consists of: Northumberland, Gateshead, Sunderland, County Durham, Darlington, Stockton, Middlesbrough, North Yorkshire and Wakefield. Main duties of the job Under the direction of a registered practitioner carry out delegated duties, work towards practicing without direct supervision and at times independently, within defined role boundaries, clinical protocols/guidelines and agreed plans of care to: Work with parents, carers, children and partner agencies to promote the physical, emotional and social development of children. Contribute to the delivery of the Healthy Child programme by undertaking some mandated contacts and targeted interventions as directed/delegated by the Specialist Community Public Health Nurse (SCPHN) and working in conjunction with the SCPHN to co-ordinate, plan, implement and evaluate a programme of care for children, young people and families. Support the completion of comprehensive health needs assessment of clients, families and community and facilitate the mobilisation of appropriate resources to meet identified needs. Ensure inter-agency/professional partnership working between services, statutory and voluntary organisations to facilitate integrated approaches in meeting the needs of children. Safeguard and promote the welfare of children, young people and adults at risk of harm by working to relevant safeguarding legislation, multi-agency policies and procedures and work within local Safeguarding Children Partnership Multi Agency Child Protection Procedures.
