This great opportunity includes working alongside peers from nursing teams across the locality, general practice, therapy and our Home visiting colleagues. To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.
