Herefordshire and Worcestershire Health and Care NHS Trust has an exciting opportunity for a motivated and dynamic Advanced Clinical Practitioner to lead a team of ACP's within the Community Hospitals of Worcestershire as a Lead ACP. The Lead ACP will possess proven leadership skills with ideas of how to adapt and evolve in an ever-changing health economy.
This is a fantastic opportunity to facilitate the standardisation of ACP roles within Community Hospitals whilst also playing a lead role in the strategic development and evaluation of advanced clinical practice. It will be vital to establish credible working relationships at all levels of the organisation and with system partners as a senior clinical practitioner therefore developing a robust workforce to support our trusts values and commitment to our patients.
This is a full-time role covering 5 of our 7 Community Hospitals. The work base is flexible but there will be a requirement to travel to each of the sites across Worcestershire.
Main duties of the job
The candidate will provide professional and operational leadership and support to Advanced Care Practitioners, undertaking annual appraisal, ensuring safe and equitable practice across the organisation.
They will be expected to work collaboratively with medical colleagues to ensure all areas have advanced clinical oversight appropriate to the needs of the clinical area.
They will provide professional, operational and clinical leadership and support for Advanced Care Practitioners or Senior Care Practitioners in training.
They will have a highly visible clinical profile, providing both advisory and clinical input into patient care, spending up to 50% of time in clinical practice.
They will provide leadership, guidance and exercise judgement in making challenging patient centred decisions within current best practice guidance, relevant legal frameworks and with clear understanding of the relevant professional and applied ethical frameworks.
They will participate in the process of recruitment and selection and induction of new staff, in conjunction with the rest of the integrated care team.

* Registered Nurse Level 1/Allied Health Professional/Paramedic/Pharmacist
* MSc Advanced Clinical Practice, which will include Advanced Assessment Skills and Prescribing V300 (if within scope of practice)
* Leadership qualification and or experience
* Evidence of a portfolio of knowledge, skills and professional development
Desirable
* Mentorship or teaching qualification / work place assessor
* Qualification(s) in long term conditions

* Demonstrable post-graduate experience in a relevant area
* Extensive post-graduate experience in a role undertaking assessments of complex needs
