Community Stroke Rehabilitation in County Durham are lookingfor a qualified Physioherapist with experience in stroke rehabilitation to joinour successful team. The available post will be based at Chester le StreetHospital, covering the surrounding areas. The successful candidate will workalongside Occupational Therapsits, Speech and Language Therapists, and skilledassociates to deliver innovative, best practice, evidence-based strokerehabilitation in patients own homes and out-patient settings to optimiserecovery.
This is an excellent opportunity for anexperienced physiotherapist to learnmore about treating stroke patients in the community in a fantastic team. Thepost is available immediately. Main duties of the job
Carry out intricate neuro assessments with patients withintheir own environments. Identify goals with patients, conduct reviews, createexercise programmes and monitor ongoing rehabilitation.
Conduct supervisory duties with other team members andcontribute to CPD
Feed into weekly team MDT

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

1 SpecialistClinical Practice:
Maintainspecialist clinical skills and provide expert advice to the area in relation toidentified specialism
To organiseand prioritise own workload and to monitor those you clinically supervisewithin a defined area
Toindependently screen, prioritise and plan interventions for routine and urgentreferrals received within the specialist area
To usesound clinical judgements to prioritise patients within a demanding andunpredictable environment ensuring effective use of resources. To assist staffunder your supervision to prioritise their caseloads and advise them onappropriate interventions in response to urgent/and or complex cases on a dailybasis
Toundertake holistic and comprehensive specialist assessments of patientsincluding those with diverse or complex presentations/multi pathologies thatchange across the caseload. To utilise analytical and clinical reasoning skillsand manual assessment techniques to provide accurate diagnosis of theircondition, these skills may be advanced; interpret facts and select theappropriate treatment and implement rehabilitation programmes
Manage crisis intervention within the community when roledemands
To assesscapacity, gain valid informed consent and have the ability to work within alegal framework with patients who lack capacity to consent to treatment
To maintaintimely documentation of clinical records and audit the documentation ofsupervisees, ensuring compliance with standards set down by the Trust, HCPC andrelevant professional bodies
Establishand maintain empathetic relationships with patients and carers in order toreassure, support and engage their involvement and co-operation in therehabilitation process and enable the giving and receiving of sensitive, oftenhighly emotional information.
Toeffectively communicate complex and often highly sensitive and emotionalinformation regarding diagnosis, rehabilitation aims and expected outcomes oftreatment to patients and carers in a manner they will understand. This will involve using a range of verbal,written and demonstration techniques to overcome any barriers to understandingcaused by physical disability (, stroke) sensory impairment (,deafness), cognitive impairment (, confusion) or emotional factors
2 Leader ofClinical Practice:
Tocommunicate effectively with patients and carers to maximise rehabilitationpotential and to ensure understanding of their condition
To liaisewith, refer to and advise the multidisciplinary team and other professionals inregard to the management and discharge of patients in your care.
To take delegated responsibility from more senior therapistin managing patients with particular conditions and be responsible forproviding comprehensive physiotherapyassessment and treatment for patients with these conditions in a designatedspecialism
