Main duties of the job
We are looking for an Advanced Clinical Practitioner (ACP) with specialist interest in Cardiology, to join an established team of Cardiology ACPs. We are looking for a dynamic and enthusiastic ACP to work collaboratively with the multi professional team in providing advanced clinical practice within local and nationally agreed protocols and guidelines, with the experience, knowledge, and leadership to develop and support the delivery of the service.
Having completed the academic and core clinical requirement (4 pillars of advanced practice), you will continue to work within ACP frameworks. The advanced clinical practitioner will practise independently to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients with acute and chronic cardiology presentations. The advanced clinical practitioner will have key relationships with all members of the nursing, medical and multidisciplinary team, as well as the leadership team.
The post holder will join the Cardiology ACP team, work a 7-day service and operates 07:30hrs - 20:30hrs working alongside our medical colleagues for inpatients through their hospital stay. This includes patients in our coronary care unit and in the ward areas.

To perform at an advanced level within the four broad based competencies of advanced practice (Health Education England 2017). Expert clinical practice, leadership and management, in order to improve and transform services. Research application of existing research and contributing to new research within chosen speciality. Education and development of self and others. The Advanced Clinical Practitioner will practice independently as a core member of the Medical Team you will have responsibility for the medical management of a defined patient or client group under guidance of the Consultant responsible for the care episode. You will demonstrate a high level of competence in specific areas of knowledge, nursing expertise, health assessment and clinical decision-making. You will provide specialist advice, education and support to patients, their carers and other health care professionals. You will practice clinically within agreed care protocols which will be monitored by the Designated Medical Lead / Practitioner. You will advise and support the Care Group management team regarding overall service strategy and future development of the service. The post holder will work across professional, departmental and Trust boundaries as required to develop and support patient-centred seamless care, with particular responsibility for leading nurse-led care initiatives.
To be successful in your application you will need to provide evidence of meeting ACP MSc academic requirements and core capabilities. We are looking for an enthusiastic, flexible individual, with good organisational and excellent communication skills. You will need the ability to work in ever-changing, demanding situations.

* Registered Healthcare Professional on relevant part of NMC / HCPC register
* MSc in Advanced Clinical Practice - full master (180 credits) Or equivalency pathway - the portfolio route - recognition by HEE centre of Advanced practice, via the supported portfolio route. Including NMP where professional registration permits. Or have formally credentialed on a nationally recognised specialty specific Advanced Clinical Practice programme for example FICM or RCEM EM ACP
* Speciality specific post registration qualification or training
* Provide evidence of leading learning in practice
* Provide evidence of supporting colleagues to promote learning in practice providing an excellent learning environment for all
