Job summary We are looking for a highly motivated Nurse Practitioner to work within the Sunderland Community Frailty Team in Sunderland. The team consists of Band 7 and Band 6 Nurse Practitioners who are based in each of our six Community Integrated Teams. You will be responsible for out of hospital care assessment and co-ordination of care and services to people who live in their own home or nursing or residential care and are identified as having moderate to severe frailty, multiple co-morbidities, complex needs or who are identified as high intensity service users. You will work as a member of the a Multidisciplinary Team and you will be an autonomous practitioner. You will be responsible for pro-actively case finding patients with moderate to severe frailty and undertaking comprehensive assessment, personalised care planning, providing support and advice to enhance self-care and improve quality of life for older people and reducing level of frailty wherever possible. You will work autonomously, assessing individual patient needs, initiating investigations, with support determining a differential diagnosis and initiating appropriate holistic, evidence based treatment and care.
