Exciting opportunity to join the Fairfax-Falls Church Community Services Board working as a nurse/case manager on the Intensive Case Management team. This team delivers community and office-based intensive case management services to seriously mentally ill individuals needing support to progress toward their recovery goals. In addition to providing office-based services, the team is designed to provide outreach, engagement, and intensive case management services in community settings such as homes, shelters, employment sites, jails, and hospitals.
This position is needed to support the ICM team. Job duties will include checking vital signs; weighing clients; doing finger sticks to monitor blood sugar; administering IM injections; and unpacking and organizing medications. Interfaces with pharmacies and insurance companies as well as prescribers, to assist clients with medication needs. Uses excellent communication skills to interact with clients with mental health, substance abuse, and/or intellectual disabilities. Duties include case management. Additional duties may be assigned as needed for clinic operations.
Location: Gartlan Center, 8119 Holland Road, Alexandria VA 22306.
Work Hours: Monday through Fridays (no holidays)
Position may be underfilled as a BHN Clinician/Case Manager in Residence; the underfill requirements are stated below in the Employment Standards section.

NECESSARY SPECIAL REQUIREMENTS:
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.
This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)
PREFERRED QUALIFICATIONS:
* Experience working with individuals diagnosed with serious mental illness and/or substance use disorders.
* Experience providing nursing care in the community.
* Experience working in the hospital or nursing home settings.
PHYSICAL REQUIREMENTS:
Ability to communicate with consumers and families with special needs in the areas of mental health, intellectual disability, and substance use disorder, and primary care. Ability to physically organize, sort, store, deliver medications and administer injections. Ability to read and record medication orders. Ability to take vital signs (blood pressure, weight, abdominal girth, temperature, pulse, blood sugar finger sticks) and use a computer to read and enter data into EHR. All duties performed with or without reasonable accommodations.
