Job responsibilities
We are currentlyrecruiting for a qualified Clinical Pharmacist to work at St Johns Surgery.
The role will be anintegral part of the day-to-day Surgery functioning with opportunities for patientfacing roles to clinically assess and treat patients using their expertknowledge of medicines alongside scope for specialist clinics and prescribing.
Theresponsibilities will include conducting Structured Medications Review and MedicinesOptimisation to tackle polypharmacy, audit and review of medicines throughNHS-led programmes such as Low Priority Prescribing and ICS/PCN driven costsaving switching.
Foran appropriate candidate, there will be potential scope for professionaldevelopment in evolving skills in independent prescribing. This will include beingresponsible for long term chronic conditions including asthma/COPD, diabetes,cardiovascular medicine and community based anticoagulant monitoring andfinally managing structured mediation reviews and medication management in thetwo Nursing Homes we provide medical care for. This will take the format ofboth telephone and face to face consulting

1. Qualified pharmacist
2. PgCert Pharmacist Independent Prescribing Qualification or equivalent

3. Relevant experience in a healthcare setting as a Pharmacist;
4. Relevant experience as a pharmacist prescriber
