An exciting opportunity has arisen for a Band 5 Nurse to work as a Mental Health Practitioner within the Assertive Outreach (AO) Service. AO is a tertiary service that takes all referrals from Secondary care services. We are looking for an enthusiastic, committed, hardworking individual to join the team. You will work under the guidance of the Senior Practitioner as a member of the MDT providing community services to adults with enduring mental health problems in their own home. You will hold a caseload of people with enduring mental health difficulties and be responsible for the assessment, planning, and evaluation of packages of care. You will require excellent communication skills and be able to communicate with a wide range of people both verbally and in written format.
Main duties of the job
The successful candidate will be a Lead professional for an allocated caseload of patients following the principles of the Care Co-ordination policy, being an integral member of the MDT, attending and contributing to MDT discussions. You will maintain accurate record keeping in line with NMC guidelines and Trust policies and procedures, coordinating and prioritizing your workload on a day-to-day basis.
The post holder will act in a professional manner and adhere to the NMC Code of Professional Conduct at all times, maintaining continuing professional development. You will actively participate in the supervision and appraisal processes, adhering to Trust policies, procedures, and clinical guidelines. Promote patient dignity, equality, diversity, and human rights and carry out duties in accordance with the Health and Safety at Work Act 1971.
