We are seeking applications from any Band 5 Nurses looking for a new challenge, or those interested in developing their knowledge and skills of our community services within the Community Health and Integrated Care Division.
The Urgent Community Response Service is a community-based team, aiming to provide health and homecare support to patients to prevent an avoidable hospital admission. As a Band 5 Nurse within the team, you will be part of a wide-ranging MDT who work together to provide a plan of care to prevent hospital admission where appropriate.
Main duties of the job
Our Nurses provide a rapid response to the assessment of acutely unwell patients at the place they call home. We continue with provision of treatment and re-assessment for patients' holistic needs under our Virtual ward. The Urgent Community Response Service also provides support to patients leaving hospital, where it is deemed that the patient no longer requires acute intervention but has ongoing short-term health needs which can be managed in the community.
The team have a daily huddle to discuss the caseload, which allows for supportive discussions around treatment provision as well as sharing of ideas and challenges. Our nursing team form part of the broad MDT that includes pharmacy, social support, AHPs, medical and speciality clinicians. As a Band 5 Nurse you will have the support of more senior clinicians available on a daily basis and are encouraged to utilise this when considering options for the management of patients on the caseload. CPD and training is offered as part of the role, as well as opportunities to be involved in ongoing service developments and initiatives.
