The Dementia Care Manager will provide expert dementia support and advice, in partnership with the regional teams, to support continuous improvement and practical help for homes regarding dementia, promoting best practice and kindness the HC-One way. They will support the implementation of the HC-One dementia Strategy and quality standards that assist those in our care lead their best lives, and contribute to the implementation of new services nationally.
Promoting a person-led, human-rights, strengths-based approach to care and balanced risk taking, the Dementia Care Manager will assist with the development and implementation of dementia care clinical policies, protocols, guidelines, materials, and pathways and provide advice and guidance on all matters relating to dementia care implementation.
Acting as clinical partners with Operational and Learning & Development colleagues they will support the delivery of our dementia care training workshops, facilitate support networks and communication methods to engage staff and promote the sharing of dementia care knowledge and best practice, supporting resident and relative involvement initiatives to ensure the customer voice is at the centre of all activities.
By supporting and advising on effective care planning and problem solving, the Dementia Care Manager will promote and recommend positive approaches to resident stress and distress reactions and will support implementation of innovative dementia care practices which are based on best evidence, and which aim to improve quality of life for residents.
This role sits within our central support function to provide cover where required.
You are a Registered nurse, or other allied health professional, with significant dementia care experience and evidence of further professional development in relation to dementia, ideally within an adult social care or multi-site care home environment.
As an established leader, you are able to act as a role model inspiring and supporting staff to deliver excellent dementia care and understand UK regulatory requirements, including mental capacity and best interests' decisions.
You have a strong focus on customer care and are a confident communicator who creates professional and credible relationships at all levels, working collaboratively to build effective working relationships both internally and externally.
The ability to travel and a full UK driving licence are required.
Safeguarding is everyone's responsibility and therefore it is important that you are able to recognise the signs which may indicate possible abuse, harm or neglect in its different forms and know what to do if there are any concerns. All colleagues are required to attend safeguarding training appropriate to their role and to undertake additional training in associated areas. You also have the ability to seek appropriate advice and report concerns, including escalation if action is not taken.
