Job Summary:
We are seeking a compassionate and dedicated Access to Work Support Worker to join our team. The ideal candidate will work with individuals who have disabilities or long-term health conditions, helping them access and thrive in their workplace or educational environment.
Key Responsibilities:
1. Support Provision:
o Provide practical support to individuals in the workplace or education settings, in line with Access to Work guidelines.
o Ensure the individual's needs are met, supporting their independence, productivity, and well-being.
2. Assessment and Liaison:
o Conduct assessments of individual needs in collaboration with the person requiring support.
o Review and adjust support plans as required, to ensure they meet the individual's evolving needs.
3. Documentation and Reporting:
o Maintain accurate records of support provided, progress, and outcomes.
4. Confidentiality and Data Protection:
o Adhere to all confidentiality and data protection policies when managing sensitive information regarding individuals and their circumstances.
5. Training and Guidance:
o Assist individuals in learning how to use assistive technologies or any other tools necessary to support their role.
6. Ongoing Support:
o Monitor and assess the effectiveness of the support plan on an ongoing basis, adjusting as necessary.
o Provide emotional and practical support to help the individual navigate any challenges that arise in their role.
Person Specification:
* Previous experience of working with individuals with disabilities or long-term health conditions.
* Good communication skills, with the ability to interact with a diverse range of individuals.
* Ability to assess needs and tailor support to the individual.
* Strong organisational skills with the ability to manage multiple tasks effectively.
* Confidentiality and sensitivity when working with personal and confidential information.
* Empathy and patience, with a commitment to supporting individuals to reach their potential.
* Experience in assistive technology or knowledge of workplace adjustments.
* Knowledge of relevant health and safety regulations and their application in workplace settings.
Working Conditions:
* Flexible working hours may be available depending on the needs of the individuals supported.
* Occasional travel between work sites.
* Some remote working options may be available.
