An exciting opportunity has arisen for an enthusiastic, motivated and committed Advanced Clinical Practitioner with strong clinical leadership and facilitation skills has arisen to join the Intensive Home Support Service (IHSS).
IHSS is an award-winning multidisciplinary team comprising ACP's, Nurses, Physiotherapists, Occupational Therapists, Nurse Associates/Assistant Practitioners and Medics. The primary aims of the service are to reduce the number of avoidable conveyances to hospital and provide a holistic and comprehensive assessment in the patients' usual place of residence. IHSS is commissioned to provide the 2-hour Urgent Community Response across East Lancashire and Blackburn with Darwen - which falls under the umbrella of Community Virtual Ward. As part of this, both remote patient monitoring for low risk/intervention patients, as well as high acuity/intervention within a Hospital at Home model is provided. IHSS currently also takes a proactive approach in generating referrals from ED via our own deflection team, as well as collaborative working with North West Ambulance Service to replace ambulance attendance for patients deemed suitable for a 2-hour UCR.
The post holder will:
• Apply advanced clinical practice knowledge and skills in providing a responsive and high level of care for patients admitted to IHSS with undifferentiated, undiagnosed illness using a range of methodologies, including physical examination, diagnostic tests and advanced health needs assessment
• Be self-directed and have a high level of skill in communication, advanced assessment, collaborative working and be knowledgeable in the subject of clinical governance and risk management as applied to advanced clinical practice and independent prescribing.
• Provide clinical line management responsibilities and professional leadership across the East Lancashire footprint, supporting the development and extension of advanced practice skills and will have a key role in embedding high quality clinical practice across IHSS with particular emphasis on advanced practice, competence, and confidence.
• Provide clinical oversight and input to patients on the IHSS caseload as well as participating in a number of collaborative projects with Royal Blackburn Teaching Hospital Emergency Department and also NWAS to maximise the 2-hour UCR capacity across Pennine Lancashire.
• Required to work across extended hours and over a 7-day period (excluding night duty).
