Job summary We are looking for a dedicated Senior Enablement Assistant to join us on our Kite Unit at the South of England Rehabilitation Centre (SERC). The Kite unit is a ten-bed neuropsychiatric rehab service for adults following an acquired brain injury, whose impairments are largely in the cognitive, behavioural, or mental health spectrum, with the aim to enable patients to rebuild their lives after a brain injury. You will be responsible for meeting the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Provides direct Enablement assistance for the patient to achieve best health outcomes. Ensures use of appropriate knowledge and skills to assess, plan, implement and evaluate patients' health needs as well as actively promote health and wellbeing. Contributes to effective discharge planning and/or referral processes to health care providers. Manage care of patients on a day-to-day basis, working under the leadership of the Nurse in Charge. Accurately documents assessments of patients on the Trust Clinical Records System. Educate and support new starters, students and support for the wider team. Role models culturally safe practice and to report poor practice when observed. Practices in accordance with legal, ethical, culturally safe, and professional standards including the Code of practice. Acts as a key worker. Plan and co-ordinates care team and planning meetings.
