North Birmingham PCN is looking to recruit a care co-ordinator to work across 6 different practices within the Erdington and Castle Vale area.
Main duties of the job
The successful candidate will be enrolled in, undertaking or qualified from appropriate training as set out by the Personalised Care Institute and work closely with the Social Prescribing Link Workers or Social Prescribing Service provider and Health and Well-being Coaches.
They will discuss patient-related concerns and follow appropriate safeguarding procedures (e.g. abuse, domestic violence and support with mental health) with a relevant GP.
The successful candidate will work in collaboration with other staff across the PCN.
Job responsibilities
The successful candidate will utilise health population intelligence to proactively identify and work with a cohort of patients to deliver personalised care, support patients to utilise decision aids in preparation for a shared decision-making conversation, holistically bring all of a person's identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person.
The candidate should have excellent written and verbal communication skills.
