An exciting opportunity has arisen for experienced RSW’s to join our Intermediate Care Team as part of the Integrated Community & Support Services (ICASS) in the Cupar Locality.
The Intermediate Care Service provides rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients, out-patients, or within their own home.
The primary function of this role will be to work with the Occupational Therapy, Physiotherapy & District Nursing Services within Intermediate Care to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists, Physiotherapists & District Nursing will provide specialist assessments to this patient group and allocate work to this post holder. This post will primarily deal with the function of daily care in the community, a service provided 365 days of the year from 0800-2200.
You will have an SVQ Level III in Health and Social Care or equivalent. NHS experience working as a support worker would be desirable. It would be desirable to have experience working with older people as they are the main group of service users referred.
Working as part of a supportive multidisciplinary team you will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
You will have good organisational skills, excellent communication skills and be able to work independently as well as within the team. There may be occasions where you will be required to work in one of the other community hospital sites. You will require to work 24.7 hours flexibly over 7 days.
Our service offers regular supervision, PDP and supports training opportunities.
The ability to travel throughout the area is essential as this is a community post.
