An exciting training opportunity has become available where we are looking to recruit a Service Coordinator to join our management team! A successful candidate will support and oversee the safe, effective and person-centred care delivered by the staff team. The applicant must have or be willing to work towards NVQ level 3 Health and Social Care.
Ideally, we are looking for someone that can offer 15 hours of Service Coordinator support alongside a Support Worker role within the service. This will require some evening and weekend working as well as some sleep ins (max. one per week). This pattern of working will enable you to get to know people really well and have the opportunity to spend social time with people you support, which is essential in a leadership role!
Lambrook Road is home to three younger adults with Learning Disabilities who have previously lived in residential care. They are all now tenants of this supported living service where they receive 24/7 support to enable them to live full and active lives. The tenants participate in a wide range of education and social activities including attending college, swimming, accessible cycling and visiting various places of interest. They need support in all aspects of their lives including personal care, shopping, cooking and maintaining their home.
The people we support at Lambrook Road do not use words to communicate so it is important that the successful candidate is able to role model creative and person-centred methods of communication to ensure that people are listened to and involved in all aspects of their life.
We are looking for someone who is creative, proactive and with a “can do” attitude. You ideally will have had some experience in leadership and management but training is available for the right person if this is your first leadership role. It is however essential that you have experience of working in a similar setting.
You will work within a patch of services so will be part of a bigger leadership team – the team works together to ensure that person-centred approaches to maximise independence, as well as high quality standards and a positive culture is set within all services. There may be times that you support others across the patch, so some flexibility is required. A successful candidate will need good IT skills to support the ongoing transfer of information onto Nourish and be able to use internal systems and processes that support daily running and reporting of the service.
The successful Service Coordinator will:
* Ensure that personalised care is provided in accordance with the values and requirements of the Trust.
* Work under the leadership of the Service Manager to ensure that service provision is delivered in a safe and personalised way, within agreed resources.
* Lead the team with enthusiasm and professionalism, being an excellent role model, taking accountability for actions and being able to reflect on lessons learned.
* Work in partnership with the recruitment team when needed including visa requirements.
* Work professionally and diligently, assisting with assessments as required and maximising the number of people supported, with referrals sought or responded to as a priority.
* Support and promote the development of colleagues working within the services, ensuring that regular reviews are conducted, and that wellbeing and learning are promoted.
