Description Suffolk County Council - Felixstowe Community Hub, Felixstowe IP11 9EF - Hybrid £33,336 per annum (pro rata for part time) 37 hours per week - Flexible working options available Permanent This advert is open to current Suffolk County Council staff only Your role and responsibilities We are looking for an imaginative, highly motivated, hardworking professional to join our team of Social Workers, Independence and Wellbeing Practitioners, Community Nurses and Therapists in an Integrated Health and Social Care Team. working with customers to identify and deliver personalised good quality outcomes to the highest standards to support and maximise their independence and wellbeing work as part of an area social work team alongside colleagues and practitioners across the Health and Voluntary sector to support effective integration between health and social care ensuring systems are joined up and accessible expected to promote Suffolk County Council’s vision, objectives, and priorities effectively to your customers, our partners, and the public and inspire others by role modelling our organisational values at all times promote a culture of value for money, outcome-based customer focus and continuous improvement. You will need a valid driving licence and access to personal transport experience of working under the Care Act 2014; Mental Capacity Act 2005 experience of undertaking safeguarding enquiries under s42 of the Care Act 2014.
