As a Specialist Palliative Care Social Worker you will work as a key member of the specialist team providing a high standard of social work, including:
Providing person centred, psychosocial support to people and their families/carers within the hospice and in the community
Upholding the highest professional standards as set out by Social Work England
Working as part of a multidisciplinary team and with external agencies, ensuring a holistic approach to patient wellbeing.
Main duties of the job
In conjunction with the other members of the Social Work team, to provide a specialist palliative care social work service for Hospice service users in the hospice and in the community.
To work as part of the multi-disciplinary team (MDT), assessing psychosocial needs of service users, and providing appropriate support and interventions pertinent to the role.
To maintain links and work in partnership with community agencies, local authorities and clinical commissioning groups for the benefit of Hospice service users.

4. Be a qualified Social Worker and registered with Social Work England

7. Have strong communication and interpersonal skills to effectively engage with colleagues, patients and their loved ones.

18. Registered with Social Work England, BA/MA in Social Work/CQS/DipSW or equivalent.
19. Working within a Multi Disciplinary Team
20. Eperience of working with individuals and families facing loss
Desirable
21. Recent/recurrent experience of fast track and continuing healthcare funding
22. Experience of supervising students
23. Experience of working in a palliative care setting
