The post holder will work 37.5hrs per week, 7.5 hours a day, Monday to Friday between the hours of 8am - 8pm. The role is based at St Marks Hospital and free staff parking is available Main duties of the job To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service Virtual Frailty Ward in collaboration with the UCR VFW Geriatrician and Specialty Doctor As an experienced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines. The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 4 healthcare/multi-therapy assistants. Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance.
