Job summary
An exciting opportunity has arisen to join the Shropshire, Telford & Wrekin Urgent Community Response & Virtual Wards Service. The post will form part of a seven day service based within one of the Trust's locality teams. The service provides patients who are experiencing an acute change in their health a safe and effective alternative to hospital admission, supported by a mixture of face to face visits and digital technology.
We are looking for an Advanced Clinical Practitioner who shares our passion for providing excellent quality of care and can devise & support the delivery of a holistic and thorough treatment plan to service users in their own home. The post holder will have full access to the Multidisciplinary Team including consultant, GP, Pharmacist, Nurses, Therapists, and Health Care Support Workers.
This Job will Support patient in the Telford locality.
Main duties of the job
The post holder is required to deliver an advanced level of clinical practice autonomously in the fields of general medicine, Frailty, Respiratory, and Cardiology for patients who are supported by the team as an alternative to hospital admission.
The role will ensure that consistent, high quality, person-centered care is provided daily through the application of advanced clinical judgments and critical thinking to assess, investigate, diagnose, prescribe, plan, implement and evaluate clinical care in line with the National Framework for Advanced Clinical Practice. Embracing the four pillars of clinical practice; leadership and management, education and research.

Training & Qualifications
Essential
* MSc qualification in Advanced Practice
* Non-Medical Prescriber (V300)
* Registered Nurse or Allied Health Professional with current NMC / HCPC registration
* Relevant qualifications or demonstrable equivalent experience in speciality (Frailty, Care of the elderly/Geriatric's, End of Life, ALS, minor injuries/illness)
* Evidence of recent CPD
* Teaching and Mentoring qualification or demonstrable equivalent experience
Experience
Essential
* Evidence of expert professional/clinical knowledge and experience within an acute and/or community setting
* Experience of working as an autonomous practitioner as an ACP, managing patients with undiagnosed clinical conditions, requiring investigations, interpretation of diagnostics, formulation and adjustment of management plans
* Evidence in involvement in supporting the development of programmes of care, protocols and clinical audit
* Evidence of relevant involvement in meeting the Trust clinical governance objective
Knowledge & Skills
Essential
* ACP underpinned by theory and experience.
* Excellent time management and organisational skills
* Ability to translate and integrate evidence-based research/guidelines into the development of pathways of care.
* Ability to manage change and contain conflict ensuring resolution with positive outcomes, with the use of negotiation skills
* Computer literate and experience of working with clinical systems
* Excellent clinical reasoning/clinical decision-making
* Knowledge of NHS Community Services
