Job summary
The post holder carriesresponsibility for the assessment of nursing care needs within our communityteam, the development, implementation and evaluation of programmes of communitynursing care and upholding our standards of care within our community nursingteams. Vitally, this role ensuresuniformity of nursing care across all community services and equity of accessto nursing care for all patients according to their needs and preferences.
Additionally the postholder is Deputy to the Head of Nursing and holds governance and reportingresponsibilities, wider strategic sight across our clinical teams and willmanage projects as appropriate.
Main duties of the job
1. Ensuringthe delivery of high standards of effective care for patients in ourcommunities with specialist palliative care needs.
2. Promotesa caring and sensitive approach to patients, visitors and colleagues.
3. Oversightof community team staffing levels and skill mix to meet the care needs of the patient population.
4. Linemanagement responsibility for Clinical Nurse Specialists, Palliative Specialist Nurses and Hospice at HomeTeam Leader, including recruitment, onboarding, professional development andappraisal, performance and sickness absence.
5. Createan environment where professional learning is actively encouraged andsupported, underpinned with a culture of continual learning and reflectivepractice.
