This is an exciting opportunity for a Practice Nurse to join our team at The Prince of Wales Surgery, Poundbury, Dorchester.
The practice is currently looking for a motivated and enthusiastic Practice Nurse with primary care experience and Diabetes chronic disease management to join our friendly and professional team on a part time basis.

Main duties of the job
Provide assessment, treatment, screening, health education services and advice to patients.
Job Responsibilities:
* Cytology
* Wound and ulcer care including removal of sutures and staples
* Day to day management of Diabetes chronic disease, maintaining disease registers, management of condition, referrals to other members of the primary care team, audit outcomes of care against standards and initiate changes as necessary
* Routine immunisations and vaccinations schedule
* Requesting pathology tests, for example urine culture, swabs, bloods
* Contraception advice including Depo and pill checks
* Administration of injections including b12
* Infection control monitoring
* Day to day use of recall systems/processes
* Venepuncture
About us
We are a 6 GP Partner Practice with a practice population of 10,000.
We have a well established, supportive and friendly Nursing Team comprising of 5 Practice Nurses, 3 Healthcare Assistants and 2 phlebotomists, working across 2 sites at Dorchester and Portesham.
Primary Responsibilities:
* Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health.
* Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
* Provide vaccination and travel medicine services.
* Provide wound care (ulcer /Doppler etc.) to patients.
* Request pathology services as necessary.
* Process pathology results as required.
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.
* Maintain accurate clinical records in conjunction with extant legislation.
* Ensure read codes are used effectively.
* Maintain chronic disease registers.
* Implement vaccination programmes for adults and children.
* Prioritise health issues and intervene appropriately.
* Recognise, assess, and refer patients presenting with mental health needs.
* Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice).
* Support the clinical team with all safeguarding matters, in accordance with local and national policies.
* Deliver opportunistic health promotion where appropriate.
* Maintain smears results logs.
* Manage emergency drug kit.

Qualifications
* Registered Nurse currently registered with the NMC.
* Experience of Primary Care including childhood and travel vaccinations, cytology, wound care.
* Experience of SystmOne Clinical System.
