We are offering an exciting opportunity for an Advanced Clinical Practitioner to join North Mersey Crisis Resolution and Home Treatment Team.
This role will require expert assessment, planning, implementation and evaluation of care for people requiring urgent support with their mental health.
This post will be based at Broadoak Unit, but practitioners will cover a footprint across Liverpool and Sefton. There may also be expectation for practitioners to support across areas of Mid Mersey depending on service need.
Key Responsibilities:
1. To provide strategic clinical leadership in relation to an area of specialism using the most up to date and evidence-based theory.
2. To undertake expert assessment, planning, implementation, and evaluation of care for a specific group of service users.
3. To offer expert clinical advice for service users, carers, staff, and other professionals.
4. To demonstrate advanced competence in highly complex decision making, assessment, and the management of clinical needs within the specialist area of practice.
5. To support and complement the existing medical arrangements.
6. To work in partnership with service users, carers, and other agencies to ensure effectiveness of outcomes.
