Job summary The Community Advanced Clinical Practitioner will contribute to the delivery of care within the Integrated community team using advanced clinical assessment skills to support timely assessment, intervention and management of the people presenting with acute, complex and / or long term conditions in a variety of settings across the community footprint / neighbourhood. The position is working in a community setting, visiting patients in their own homes to support them to avoid unnecessary hospital admissions. You will drive the ACP agenda and support the clinical lead to develop the service for our patients. The successful candidate will ensure they achieve all 4 pillars of advanced practice. Predominantly they will undertake clinical assessment of complex older people, aiming to reduce unnecessary hospital admissions. They will support he virtual ward and the urgent community response service as well as working with the Primary Care Network. Hours per week: 22.5 Interview date: 5th March 2025 Main duties of the job The Community ACP will: - Undertake assessment of patients with differential and undiagnosed illness or exacerbation of existing conditions using advanced clinical skills, critical thinking, decision making and risk analysis, to provide safe, high quality, evidence-based care preventing unnecessary acute hospital admissions, supporting patients in their own homes for longer and preventing premature admissions into long term care facilities Provide advanced clinical assessment of patients including appropriate diagnostics, early interventions to determine a clear clinical management plan and ensuring that the right care is received, in the right place at the right time. Take a lead clinical role within the community team supporting the triage and streaming function advanced clinical assessment and as part of the integrated team.
