Main duties of the job
We currently have an opportunity for a Community Band 6 nurse to join our team, working in the London Borough of Lewisham. This role is a part-time role, covering the Twilight (out of hours) service, which runs from 5:30-10:30pm every evening.
This role is open for a 6 month secondment, looking to start on or around 2nd April 2024.
The successful candidate will be experienced in caring for patients with a variety of long-term conditions and, with a desire to develop their leadership skills. Applicants will have the opportunity to apply their knowledge and evidence-based practice to community care.
The aim of our service is to enable patients to stay at home longer, and leave hospitals faster, maximising their potential for independence and wellbeing.
As no two days are the same in the community, you will be working with a large degree of autonomy and accountability, with the care you are giving making a real difference to your patients and those important to them.

To succeed in our aim for patients to stay at home longer, our community teams work closely with members of the wider multi-disciplinary teams to plan and deliver complex care.
