As a Care home Care Co-ordinator, you will work as part of the Enhanced Health in Care Homes team in Darlington. You will play an administrative role in co-ordinating care planning and support for residents in CQC registered care homes in the town. You will have responsibility for arranging meetings of the Multi-disciplinary teams supporting residents in care homes. You will liaise closely with GPs, Practice staff, community nursing staff, and other NHS professionals, acting as part of a wider multi-agency integrated team.
This role involves working with residents and teams in care homes to ensure that the care is aligned to residents' needs. It is to ensure equitable care. This is an interesting job opportunity for an experienced administrator to make a difference to care home residents.
In this position, you will be employed by Primary Healthcare Darlington on behalf of Darlington PCN in a role funded through the Additional Roles Reimbursement Scheme (ARRS) and the General Practice Network Contract Directed Enhanced Service (2024/25) (the DES). As part of your role, you will work closely with the PCN and the General Practices across Darlington.

Responsibilities
* To liaise with care home managers to identify those residents who would benefit from having a review.
* To organize regular meetings of Multi-disciplinary teams (MDTs) supporting each care home and the EHICH clinical team.
* To ensure all residents receive a comprehensive geriatric assessment.
* To ensure care plans are completed for each resident.
* To foster good communication between care home managers and all members of the MDT.
* To maintain and develop engagement with GPs, practice staff, and community care staff.
* To liaise with multi-agencies to coordinate pathways of care for residents.
* To help residents manage their needs, answering their queries and supporting them to access primary care services.
* To ensure residents have good quality information to enable them to make choices about their care.
* To collect data on residents for recognised outcome measures and ensure residents' notes and care plans are updated to reflect any changes.
Person Specification
Qualifications
* GCSE/NVQ level 2/3 or equivalent level of training and experience.
Experience
* Understanding of the NHS and how it works.
* Proven ability to work with and communicate confidently with differing levels of staffing.
* Proven ability to work with external stakeholders.
* Good interpersonal skills.
* Ability to work well across teams.
* Good time management and an ability to prioritise and work to strict deadlines.
