Here's what you'll bring to the role: - Recent clinical or ward pharmacy experience preferred. - Strong multidisciplinary teamwork experience. - Familiarity with respiratory, surgery, or elderly care specialties (other specialties will also be considered). - Hospital pharmacy experience is a plus, but we also offer a Band 6-7 development programme for candidates looking to grow. Main duties of the job - Deliver expert clinical pharmacy services, including prescription reviews, patient counselling, and therapeutic drug monitoring. - Provide guidance on the safe, effective, and cost-efficient use of medications while ensuring adherence to Trust policies and professional standards. - Collaborate with multidisciplinary teams to develop tailored pharmaceutical care plans and optimize patient outcomes. - Participate in departmental audits, policy reviews, and service development projects. - Support the education and training of pharmacy team members, students, and healthcare professionals through mentoring and formal teaching. - Contribute to continuous improvement by implementing evidence-based practices and participating in governance meetings.
