The Community Hub The Community Hub at Leckhampton Court Hospice offers day services to adults with a life limiting diagnosis. We support patients, their carers and loved ones, focusing on wellbeing, quality of life and rehabilitation. We offer a growing range of programmes, workshops and group sessions, as well as individual appointments in our outpatient clinics, and Complementary Therapy service. We work collaboratively with our Wellbeing, Therapies, Befriending, Hospice at Home and Inpatient teams and are often the first point of contact for patients engaging with the hospice.About the role The successful candidate will be working predominantly in our Community Hub. You will be assessing individual needs, to create bespoke care plans. You will monitor patients and evaluate care as part of the role, working collaboratively with other teams within the hospice, and the wider healthcare community, to ensure that we are responsive as their needs change. You will work closely with the Community Services Manager and will have the opportunity to contribute to the development of the evolving service, including new programmes, workshops and support groups. You will facilitate group sessions, with the support of the multidisciplinary team, managing and encouraging group dynamics, providing support, and using excellent communication skills to enable Advanced Care Planning and End of Life Care discussions. You will be required to use your nursing experience to provide the highest standards of evidence based care.
