Detailed job description and main responsibilities The Overnight Team's primary aim is hospital admission avoidance and keeping patient sefley in community. The team reviews patients with worsening medical and functional conditions within their own home and liaise with the wider community teams to prevent clinical deterioration and admission to acute hospital. Please see attached Job description and Person Specification Person specification Education & professional Qualifications Essential criteria Registered Nurse with current NMC registration Evidence of continued professional development Independent Nurse Prescriber (V300) Experience Essential criteria Experience of working within the Community Nursing Extensive post registration experience in Acute care Experience in ITU, A&E, Acute assessment units would be advantageous Skills and aptitudes Essential criteria Ability to assess and implement appropriate care plan and adapt plan depending on patient's response to treatment. Ability to assess and identify the deteriorating patient Able to work confidently in an independent and autonomous role whilst being aware of own limitations Employer certification / accreditation badges Applicant requirements You must have appropriate UK professional registration.
