Paramedic Practitioner - Horsham Central PCN
The post holder will work across the practices with each of the practice teams, joining the PCN team for Central Horsham. They will be assigned a lead clinical and operational contact for each contract. ABC as employer will provide support and ensure access to the NHSE England training programme as required.
This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England.
The paramedic practitioners will take referrals direct from practices to carry out in practice clinics, home visits, telephone or online triage as appropriate. The role will involve the assessment, diagnosis, treatment or referral of patients presenting with undifferentiated and undiagnosed conditions. The post holder will work within their level of competency and will be clinically supervised by a General Practitioner working in the area.
This role will have completed or be working toward a Band 7 First Contact Practitioner (Paramedic) role, based on the completion of stage 1 and stage 2 of the Health Education England Competency Framework. You will then be assisted to develop and enter the ACP pathway. You will be supported through indirect supervision and will be responsible for assessing, diagnosing, interpreting investigations, and treating patients within Primary Care within a developing scope of competence.
Main duties of the job
You will monitor and assist with improving standards of care utilising clinical expertise and experiences. You will teach and support colleagues and students where appropriate, providing professional clinical leadership. Where appropriate you will act as an expert in your field and lead and/or participate in initiatives to improve standards of care for the client group and assist in advancing the ACP role.
The trainee Advanced Clinical Practitioner is service driven, and you will be developing around the Department of Health Guidance across the four domains of Advanced Practice, those being:

* Professional leadership and Consultancy
The Advanced Clinical Practice MSc course can be completed through Brighton University or the Open University. The PCN welcomes all applicants who meet the criteria for the Trainee ACP role and will support candidates who are at different stages of completing the FCP roadmap.

Job responsibilities
Clinical Responsibilities
* To provide assessment, diagnosis and treatment at first point of contact by attending to patients according to patient needs either in the Practice or patient's home.
* Work with practice teams to ensure the process for referral of urgent on the day patients is smooth and appropriate patients are referred making full use of the practitioners skills.
* To accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
* To evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
* To undertake and interpret diagnostics and take appropriate action to manage patient care, whilst distinguishing between abnormal and normal findings in order to relate them to a possible diagnosis. To use broad clinical guidelines (NICE etc) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.
* To signpost appropriately to other services in the identified healthcare pathway.
* To refer patients to appropriate care settings, within locally agreed healthcare pathways.
* To advise and support members of the public on healthy lifestyles, health promotion and disease prevention.
* To maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.
Communication
* Ensure via use of clinical systems, attendance at meetings and other means that are a core part of the practice MDT supporting patients and work with the wider MDT including staff from SCFT, SPFT, Local Authority and Voluntary Sector organisations.
* Ensure high quality, clear communication with patients and carers including within challenging situations.
Professional
* To proactively support audit of own caseload and that of colleagues to ensure the quality of the service is maintained and continuously improved. To seek out feedback from other members of the MDT and patients to support this.
* To undertake all necessary training and be familiar with all policies to ensure that comply with expected procedures and ways of working.
* To ensure practice is within good clinical governance and based on sound up-to-date clinical evidence. Where appropriate, to identify and assess any risks associated with patient care or to oneself and communicate these risks to the appropriate clinical manager.

Experience
* Ability to manage and prioritise workload

* Previous experience of non-medical prescribing and using PGDs
* Experience as an autonomous practitioner within a minor injury/illness centre or mobile service
* Experience of the ECP/Advanced Practitioner role within Primary or Secondary Care
