Our Enablement and Urgent Care services are responsive and flexible with the aim of supporting people to develop and maintain their independence within their home environment. Our aim is to prevent unnecessary hospital admission and facilitate early discharge from the acute hospitals.
The Home First Team provides short-term enablement support and assessment of ongoing care needs to enable people to be as independent as possible in their home environment. The service is a KCHFT and KCC Adult Social Care Partnership.
Please note, the ability to drive in the UK and access to a vehicle are essential for this role.
Providing support and encouragement with activities of daily living, including personal hygiene needs & toileting, to people recently discharged from hospital and promoting and supporting the person to be as independent as possible in their home environment.
We are looking for staff who:
1. Believe in providing personalised support.
2. Can work independently and flexibly.
3. Respect privacy and dignity and who want to expand their knowledge in both a Health and Social Care setting.
4. No experience in the Health and Social Care sector is required, however, the ability to work flexibly and a passion to make a difference is a must.
Additional Working Pattern: 13 x 11.5 hour shifts over 4 week period. Pattern to be confirmed. Requires alternate weekend working.
