Job responsibilities Provide excellent administrative support to the Frailty team. Communicate with patients face to face, via telephone, online and email, both in the community and within practice. Work with colleagues in the frailty team to identify patients who would benefit from proactive review. Work with patients, carers, and families helping them to organise appointments, arrange support and signpost relevant services. Work alongside a multidisciplinary team to ensure comprehensive and cohesive patient care. Liaise with community partners such as district nursing, palliative care teams, voluntary sector organisations, the integrated care therapy team to improve patient and carer outcomes. Liaise with secondary care services when needed to ensure continuity of care for our patients, including supporting with arrangements following hospital discharge. Use PCN software to record accurate information and run reports. Uptake any training required, as directed by line managers and contractual requirements.
