To participate as an effective member of the MDT and be responsible for the development and implementation of a patient-centred approach to care: assessing, developing, implementing, and evaluating the care needs of patients and their carers. This includes meeting the physical, psychological, social, and cultural needs of the individual and their family, using evidence-based knowledge and skills to maintain the individual's identity.
Apply clinical knowledge and experience to provide education for patients to develop a greater understanding of their mental health condition, working in a patient-centred way with clients to help develop their awareness of their condition, life skills, and coping strategies. Recognise the needs of the family/carers, ensuring their involvement in care where appropriate while offering support and education with empathy.
To undertake initial mental health and risk assessments of children and young people (CYP) who present with high risk and/or following acts of deliberate self-harm. Independently and/or jointly carry out assessments of the mental health needs of children and young people within a therapeutic framework and contribute fully to the care planning process.
Provide a high standard of care for people in the care group in keeping with the philosophy and operational policies for the Liaison Team, in addition to the Trust's policies, practice, and procedures. Conduct accurate risk assessments and management plans, involving the service user, carer, and any other individuals relevant to the care of the service user, ensuring thorough documentation (on RiO and the acute hospital EPR).
Participate in network and discharge planning meetings to coordinate and identify appropriate services following crisis presentation. This may involve participation in the transition process to Adult Mental Health Services where appropriate. Be responsible for negotiating coordinated rehabilitative care packages, in conjunction with other professional agencies and networks, prior to discharge from the acute hospital back to the community.
Identify and respond appropriately to child protection issues within local, Pan London, and national Safeguarding Children guidelines, including participation in case conferences, review meetings, and planning meetings as appropriate. Link resources and coordinate services for young people and their families experiencing disruption due to severe mental illness.
Ensure, in conjunction with the Community CAMHS team clinicians, the follow-up of children and young people who attend hospital following episodes of suicide attempts, self-harm, and other acute mental health presentations. Ensure effective communications with relatives, other professionals, and voluntary organizations, thus encouraging participation in care delivery. Respond sensitively to the needs of relatives and carers.
Develop strong links with the CAMHS Teams, as well as other professional groups/teams. Provide specialist consultation to the families and others involved in the care of the young person. Assist with the management of the behaviour of children and young people who may be physically and verbally aggressive towards other members of the family and staff.
Offer advice to other professionals within CAMHS and externally, and share expertise in the management of complex cases in various settings. Communicate in a clear and sensitive manner information concerning the assessment, formulation, and treatment plans for service users, and monitor progress during the course of multi-disciplinary care.
Offer short therapeutic interventions to patients and their families with the therapeutic modality in which you have experience.
